Completing the Survey Online

 

Adding Establishments/Locations

 

Additional establishments can be added through your Form NC-99### accessible from the Main Menu.

 

Adding Establishments/Locations

 

To add additional establishments, complete the NC-99###: Ownership and Control form and click "Next" to be taken to the Additional Establishments screen.

The "Add Establishment" button allows you to add establishments one at a time. To add an establishment, click the button, complete the next screen with the information for the establishment you are adding and hit "Save."

Your added establishment(s) will appear in your Additional Establishments list. Once you have added all your establishments, click "Next," enter any remarks, and click "Next" to be taken to the NC-99### Form Review page.

 

Please make sure the number of establishments reported in NC-99### Item 1 matches the number of establishments listed on the Additional Establishments screen.

Deleting Added Locations

 

You can only delete locations added through the Additional Establishments feature. To delete an added establishment, use the "Delete" button on the Additional Establishments screen.

Please make sure the number of establishments reported in NC-99### Item 1 matches the number of establishments listed on the Additional Establishments screen.

 

If you wish to remove your prelisted establishment, then update Item 3, Operational Status as ceased operation or sold.

Error and Warning Flags

 

Errors and Warnings are components of the survey controls. They allow you to edit your data before sending it to the Census Bureau. You may get an Error or Warning message if questions are left blank or incomplete. You will not be able to submit forms without first correcting all Errors. Before you navigate past the Item that has produced an Error or Warning, an alert message will appear at the top and bottom of the Item. The alert message will detail the issue and provide guidance on how to correct it.

Once the Item is corrected and navigated away from, the alert message will be removed. The Form Review screen will also display Items with Errors or Warnings. Please see Reviewing a Form for more information.

How to Report Dollar Figures and Percents

 

Dollar figures should be rounded to the nearest thousand dollars as illustrated in the example below.

Percents should be rounded to the nearest whole percent as illustrated in the example below.

Main Menu

 

The Main Menu is the home page for completing your survey online and shows all your survey forms.

This page allows you to Open your report forms, view the Status of your forms, view the OMB number and expiration date, see the Due Date, and a summary of Problem Pages. You can also View/Print your forms from the Main Menu

Opening A Form

 

To open a form from the Main Menu, click the appropriate button in the Action column. The button label will change from "Start" (if this is your first time accessing the form) to "Resume" to "Review" depending on the status of the form.

Printing A Form

 

This option allows you to print a copy of your survey forms.  The printout will replicate the corresponding paper report forms.  You can print any form at any state of its completion by clicking the PDF icon in the View/Print PDF column on the Main Menu.

Then select "Open" (to view or print) or "Save" (to save the file to your computer) in the dialog box.

Reviewing A Form

 

Once you have completed your form, you will be able to review your responses for Errors and Warnings on the Form Review screen. You can access the form review screen by completing all Items and clicking "Next" on the Remarks page or by clicking the "Review" button in the Action column on the Main Menu.

If applicable, the Form Review screen will display an icon that shows that the form contains Errors (and/or Warnings) that will need to be corrected prior to submitting the form.

Click the link in the Item List for the Item you selected.

This will take you directly to the Item you selected. Any Error or Warning messages will be displayed.

Saving A Form

 

Your data is automatically saved whenever you use the navigation buttons found at the bottom of each Item screen.

Note: Logging out of the survey will not save any data you have entered on the current page. To save and log out, you must first navigate away from the current page by using the "Next," "Previous," or "Main Menu" buttons and then use the "Logout" button.

Submitting Responses

 

Once you have completed all your forms and cleared all errors, you may certify and submit your responses.

 

Note: All forms must be submitted at the same time.

 

You may submit your forms through the Census Bureau website by following these steps:

 

1.  Click on the "Submit Forms" button located on the Main Menu page.

Note: The "Submit Forms" button will be disabled if you have errors in the form(s) or if a form has not been completed.

 

2. Complete the Certification screen and then click the "Next" button. The Submission Confirmation page will be displayed showing the forms were submitted successfully.

Toolbar

The Toolbar is available on all pages within the Direct Internet Reporting site. The Toolbar provides links to Logout of the system, Instructions, how to Contact Staff, and Help with completing the report.
