Once you have responded to the online survey(s), you may print a PDF copy of your responses for your records; However, you do not need to mail or fax it to the Census Bureau.

If you receive a report form in the mail after you submit your data online, please disregard. It is possible the forms may have crossed in the mail. You can verify your company's status by signing in Centurion and going to the Main Menu.
