To access requests:

1. Have your User ID (uid) and Password (pw) handy. The password is case sensitive.
   These can be found in the label of your report form. They are also shown in the mailing label of letters sent to multi-establishment companies.
   
   
   
   
2. Go to our secure Login Page
   

Please note that the password is case sensitive

3. You can do the following online:
   
   
   • Filing Status
     Check which report forms we have received from your company and
     which report forms we have not received from your company.
     Also check your company calender of surveys.
     
     
   • Remail Forms
     Order replacements for lost or misplaced report forms.
     
     
   • Time Extensions
     Request additional time to complete your report forms.
     
     
   • Decryption Key / Passphrase
     Use this key if you received an encrypted CD-ROM.
   
   
   
   • Secure Messaging Center (SMC)
     Securely exchange messages and data files with Census Bureau staff.
