Instructions

    1. Locate your user name (UID) and password (PW) on the front page of the questionnaire booklet mailed to you in January 2009.
      Note: The password is case sensitive.
    2. Click the following link to open a new browser window and log in to the web-based survey: Log in to survey.
    3. You may stop at any time and return later to complete the survey.
    4. Once you have responded to the online survey, you may print a copy of your responses for your records.
